How to send/submit official documents:

[ə-fĭsh′əl] adjective

Documentation that is ordered and delivered from one institution's registrar to the other.
Must be ordered by the student-athlete from the office of the registrar, records, or admissions.
Records must come from the institution where the hours were earned.
(i.e. OUAZ cannot send official transcripts received from other schools to the NAIA)

Official Documentation must be sent to BOTH the NAIA Eligibility Center & OUAZ.

ALL transcripts from ALL institutions are required to be sent to both locations.

* if you are a sport that does not require an NAIA determination, only send documentation to OUAZ.

Please use the form linked below to send an

unofficial copy (screenshot or pdf print)

of your ACT or SAT scores to OUAZ.

Test score & breakdown

Frequently Asked Questions:

Test Scores through High School:

If you took your ACT or SAT through your High School, you should still have a login to order official test scores through the testing organization. Ask your high school about the log-in information.

Provide Receipts for Review:

If you believe that you have already sent official transcripts to OUAZ, please provide your coach with a receipt of when you ordered, how they are being delivered, and where they were sent.

Admissions vs Athletic Eligibility:

Unofficial transcripts can be used for admissions, but official transcripts are required for athletic eligibility.
Be sure to send all official transcripts to the OUAZ registrars so that they remain official. Admissions will be able to access them if they are sent to the registrars!
